Something wrong about directed-broadcast command.
Router A: " ip directed-broadcast " shouldn't put in ethernet0.
Router C: " ip directed-broadcast " should put in ethernet0 but
not ethernet1 suppose ethernet0 connect subnet 178.21.34.0. 
And ethernet 0 don't need in multicast mode.

Router A

interface ethernet 0 
 ip multicast helper-map broadcast 224.5.5.5 120 
 ip pim dense-mode 
! 
access-list 120 permit udp any any 4000 
access-list 120 deny udp any any  
 ip forward-protocol udp 4000 

Router CLast Hop Router Configuration

 ip directed-broadcast
